Hi,I am Dr. Shanthi.E (General & Family Physician). I will be looking into your question and guiding you through the process. Please write your question below.
c.t.scan of lower abdomen post contrast helical ct scan of lower abdomen was performed ,plain sections were also obtained . delayed scan was taken for bladder. urinary bladder is partially distended. no evidence of calculus seen. mild anterior bladder wall thickening is seen ( approx.3-4mm).posterior and lateral bladder walls appear relatively normal. prostate is enlarge in size , measuring approx. 4.3cm×4.8cm×4.9cm(volume 50cc) with slightly heterogeneous central gland enhancement.periprostatic fat planes are clear.
The report of CT scan of the abdomen & pelvis is indicative of benign prostate enlargement (as periprostatic fat planes are clear) with secondary changes in the urinary bladder due to obstructive uropathy. As per my surgical experience, primary care of this issue is with selective alpha blockers as tamsulosin hydrochloride to reduce the size of prostate over a period of time (around 3-6 months).
Later on decision of surgery as per the symptomatic improvement or early if the patient wishes for laser removal of prostate or endoscopic fulguration via trans-urethral approach. However, there are always options of emergency indwelling catheterization in case of acute retention of urine under sterile techniques by consultant surgeon.
Hope I have answered your question. Let me know if I can assist you further.
Regards,
Dr. Bhagyesh V. Patel, General Surgeon
